    • UPIC – Track Selection

Only Medicare patients should complete the Annual Medicare Wellness Questionnaire.

ALL forms must be completed prior to being scheduled for your initial appointment.

Insurance

  • If you are using insurance, contact your insurance company to make sure we are contracted with your specific plan. You can call the customer service number on your insurance card and ask if Karen Steed is contracted with your plan.
  • If yes, please email a copy of the front and back of your insurance card to office@findinnovativehealthcare.com after you complete the above forms.

Self-pay

  • If you are self-pay, we do have standard visit rates.

Note

  • Karen has elected NOT to prescribe controlled medications such as Testosterone, Xanax, Ambien, Phentermine, and Hydrocodone. There are no exceptions to this. If you are unsure about one of your medications, please contact your pharmacy.

Hormones

  • If you are being seen for hormones, please note that we measure hormone levels via saliva testing which is an out-of-pocket expense. We do not use pellets.
